Recovery and Support Coordinator - Burlington/Skagit Recovery Center

The CCS NW Recovery Centers provide a full continuum of outpatient Substance Use Disorder treatment services.
CCS provides treatment to youth, adults, and their families.

The philosophy of CCS NW Recovery Centers is that treatment is most effective when it is family-centered.
The Recovery & Support Coordinator position was developed in conjunction with Skagit County Human Services & HCA.
Incumbent shall provide State Opioid Response (SOR) recovery support services to eligible individuals.
Eligible Individuals include people with a diagnosis of an Opioid Use Disorder (SOR) or individuals with a demonstrated history of opioid overdose problems.
The purpose is to provide client-directed recovery support services and to encourage and support their continued recovery efforts.
Position is:
Full-time Status - (37.
5 hours per week).
Pay is:
$17.
85 - $18.

Compensation is set by agreed to language in the Collective Bargaining Agreement between CCSNW & SEIU 1199NW and determined based on experience.
Note to internal candidates:
HR reviews internal compensation and determines increase based on their experience and also agency internal equity factors.
MAJOR DUTIES AND RESPONSIBILITIES
Connect with Skagit County providers and coordinate across service systems to develop SOR program resources, such as:
housing, treatment, law and justice, medical, education, employment, crisis intervention, transportation, basic needs etc.

Establish a referral process for community Medication Assisted Treatment (MAT) providers to refer individuals receiving MAT to recovery support services.

Identify, develop, and maintain partnerships with organizations/vendors that are key to promoting the individuals health, recovery and continued goals.

Identify individuals and families who would benefit from being served through the SOR program.

Provide screenings and intakes for community referred clients to assess if they are a good candidate for the SOR program
Provide client directed recovery support services such as recovery planning, peer services, recovery coaching, support groups and education, basic needs, transportation and medical support.

Provide access to goods and services that are recovery supportive, client driven, and include family/community engagement and involvement to support recovery efforts.

Work with the identified SOR recipients to develop an Individualized Recovery Plan (IRP); including identifying services needed to support continued recovery, collaborate with providers and link clients to identified resources.

Complete the federally required GPRA packet with all RSP recipients at admission, 90 days, 180 days, and discharge.

Ensure data entry of all GPRA packets are entered into appropriate databases within the state required time frames.

Ensure that 80% of individuals that receive recovery support services complete a three (3) month follow up and a six (6) month follow up GPRA survey.

Issue funds for specific basic needs or community wellness classes/programs that are supporting the clients IRP.

Hold clients accountable for improper spending of funds.
Follow through with corrective action plans and possible termination from the program is needed.

Provide Skagit County with quarterly reports as defined in the contract.

Provide engagement services that demonstrate effectiveness through measurable outcomes.

Work closely with the Billing and Accounting staff to ensure all required forms are completed correctly and proper records are maintained.

Track the individuals' monthly spending to ensure adequate funds throughout the duration of their program duration, not to exceed $5,700 total per individual.

Implement any changes that become established for the program in regards to processes, procedures or changes to documentation requirements.

Collaborate with leadership to continually maintain/evaluate program forms, processes and procedures
Provide flexibility in service hours in order to serve as many clients as possible.

Works from designated off-site location as a program requirement such as at substance abuse provider agencies and community settings.

Attend community and other program related meetings as required.
Access to and protection of PHI:

Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area).

Complies with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms).
Perform other work-related duties as assigned.
MINIMUM:

BA in Social Services or related field or four years relevant SUD experience.

Experience working with individuals with substance use disorders (SUD), specifically opioid use disorders (OUD).

Experience with creating and maintaining collaborative relationships.

In-depth knowledge of viable community resources.

Excellent organizational and time management skills with the ability to schedule and manage workload sufficiently to meet deadlines.

Excellent interpersonal skills with the ability to communicate clearly (both in writing and orally).

Proficient with data base entry.

Ability to work independently, as well as represent the Agency in a professional and proactive manner.

Current WA State Driver's License & proof of current automobile insurance and reliable transportation.

Proof of acceptable driving record (Abstract-Employment Version from Dept.
of Licensing).

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
